Bariatric (Obesity) surgery is an option in severely obese patients, where lifestyle modification, exercise regimes or medications have been tried but found not to be effective.

Bariatric surgical procedures work by reducing intake or absorption of calories. There are various procedures, all of which have been effective depending on patients profile. Bariatric surgery should always be performed in a specialist centre, and long-term follow-up of patients is necessary.


Why people become obese?

Here's a very simple explanation of how you you put on weight and will lose weight with surgery. Your body needs energy to function and your energy source is the food that you eat. A normal weight person successfully balances their energy intake (food eaten) with their energy requirements (daily activity). In other words, they burn up all the energy provided by the food they eat, and stay slim. However, you began to get fatter when you started to eat more food than you needed for your energy requirements. Over a long period of time the excess has been stored as fat around your body. In theory, just hundred calories eaten more than you need each day would lead to a weight gain of five kgs per year. A person who is roughly thirty kgs (sixty five lbs) or more above their ideal weight is considered to be severely obese. However, obesity is a complex condition and not simply caused by overeating. In fact, many people eat more calories than they need, but not everyone stores the excess as fat. There seems to be some sort of in–built mechanism that causes excessive fat storage in some people, and scientists are working to try and understand more about it. Severe obesity tends to run in families so there is likely to be a genetic component as well

What is Obesity or Bariatric surgery?

Bariatric surgery, also known as weight loss and obesity surgery, refers to the operations designed to reduce weight (Bariatric come from the Greek meaning pressure or weight). Operations may restrict the amount you are able to eat (restrictive operations such as the gastric band sleeve Gastrectomy) or reduce the amount of food you can absorb or both restrict the amount you can eat and reduce the amount you can absorb (gastric bypass or duodenal switch). The term does not include procedures for surgical removal of body fat such as liposuction or Abdominoplasty (tummy tuck).


Why treat obesity?

The costs linked with treating obesity and its related health conditions are enormous. Putting the costs aside, the main concern regarding obesity is the impact it has on health. Almost all body systems are affected by obesity. Being overweight can also shorten your life. On average obese people are likely to die ten years earlier than those who are not overweight, preventing many from reaching retirement age. The heavier you are and the longer you have been overweight the greater the risk.

How surgery works

The human body lays down fat stores if the calories in the food you eat exceed the energy you burn up. There are a number of complex processes which the medical profession are only just starting to understand that seem to contribute to increasing obesity - the appetite mechanisms go wrong and you continue to eat even though you do not need the calories. And of course, once your body is carrying the extra weight, it becomes harder to be physically active which would help to regulate your weight. Obesity surgery works by helping to reduce the number of calories that are available in your body.

There are two ways this can be achieved surgically

Restriction - by reducing the size of the stomach, only small meals can be eaten and the appetite is satisfied.

Malabsorption - by bypassing part of the small intestine, less calories from food are absorbed by the body.
